The Whole Person Plan: A Tool to Guide APRNs in Palliative Care Discussions and Assessments is a clinical tool you can use to clearly describe palliative care to patients and families who need it, and to aid in ongoing assessment, discussion, and palliation of suffering in the face of serious illness.
The Whole Person Plan highlights the physical, social, emotional, and spiritual domains essential to effective whole person care, and the importance of future planning, which helps patients articulate and document healthcare decisions that align with their goals and values.
This tool was created by Kendra Deja, MSN, GNP-BC, ACPHN, LCSW, in collaboration with the CSU Shiley Institute for Palliative Care. The eBook includes a downloadable version of the Whole Person Plan, as well as a plethora of evidence-based resources that you can use to assess and address suffering in each domain that comprises the whole person.
