As a CSU faculty or staff member, if you intend to pay with your university credit card or be reimbursed by your campus, payment must be a transfer of funds between your campus and CSUSM. DO NOT PROCEED TO REGISTER BY CLICKING THE PROFESSIONAL REGISTRATION VIEW CATEGORY BUTTON BELOW. The questionnaire to register is necessary to support the transfer of funds from your campus to CSUSM. *CSU FACULTY & STAFF ONLY, please click the ‘CSU Only Faculty & Staff’ button below to purchase the recording via a questionnaire. You will be required to pay for the recording with an accounting journal entry between your campus/department and the Institute. Instructions on how to navigate this method of payment will be provided once you fill out the questionnaire. This is ONLY for if you want to be reimbursed and/or using a pro-card.

Terms and Conditions for Registration
Refunds and Cancellation
If you are unable to attend the Symposium, you must send written notice of your withdrawal via email to the Institute at palcareadmin1@csusm.edu at least five (5) business days before the first day of the event for a full refund. Refunds will not be issued after the Symposium begins. Registrations are nontransferable. Refunds will be issued 30–45 days following the Symposium.
If the CSU Shiley Haynes Institute for Palliative Care (Institute) cancels or substantially alters the event (e.g., moved to virtual format) due to unforeseen events, natural disasters, or local pandemic conditions/restrictions affecting the ability of attendees to travel or gather safely, the Institute will refund.
Privacy
We value your privacy. Your personal information will be kept confidential and will never be sold to third parties. It will only be used for communications related to your attendance at the Symposium, and to related services provided by the exhibitors and supporters present at the Symposium. Photography
By attending the Symposium, you acknowledge that photographs and/or videos of you may be taken by our conference staff and/or photographers at any time. Furthermore, you grant the CSU Shiley Haynes Institute for Palliative Care permission to use photographs and/or video of your likeness in any type of media, including websites and print publications, without compensation or reward.
Alcohol, Smoking, Drugs
The consumption of alcohol, smoking &/or vaping of tobacco and any other substance, and use of drugs are prohibited anywhere in/on the university campus, including parking areas. Consumption of alcohol is OK only if included in an on campus approved special event. The Institute reserves the right to remove any person or persons from the premises for violation of this policy.
Nondiscrimination
Attendees agree not to unlawfully discriminate against any person based on race, color, national origin, religion, sex, pregnancy, marital status, sexual orientation, gender identity, age, physical or mental disability, covered veteran status, or any other protected status.
Disclaimer
The CSU Shiley Haynes Institute for Palliative Care, its planning partners, and supporters will make their best attempt to present the program as presented on this website. We reserve the right to alter without prior notice, any of the arrangements, timetables, plans, speakers, or other items related to the Symposium, for any cause beyond its reasonable control. The CSU Shiley Haynes Institute for Palliative Care, its planning partners, or supporters are not liable for any loss or inconvenience caused as a result of such alteration.
Attendee assumes the risk of injury to person or property, or death, in connection with travel to and from any site(s) related to participation in the symposium. Attendee has sole responsibility for personal possessions during the symposium and any related activities. Attendees are urged to secure themselves against risks of liability or loss.
In the event of unforeseen circumstances, the CSU Shiley Haynes Institute for Palliative Care, its planning partners, or supporters do not accept responsibility for loss of monies. Participants are advised to take out personal travel insurance and to extend their policy to cover personal possessions. The CSU Shiley Haynes Institute for Palliative Care, its planning partners, or supporters do not cover individuals against cancellations of bookings or theft or damage to belongings.
Disability Accommodations or Special Needs
Individuals with disabilities, who would like to attend this event, please contact the Symposium Coordinator, Cynthia Runge, at crunge@csusm.edu regarding any special accommodation needs. It is requested that anyone who requires auxiliary aids such as sign language interpreters and alternative format materials notify the event sponsor at least ten business days in advance. Every reasonable effort will be made to provide reasonable accommodations in an effective and timely manner.
